This means we will begin your first visit with us by gathering information about you and your medical history.
What to Bring to Your First Appointment
For your convenience, we provide new patient forms right on our website that you can download and fill out ahead of time. Be sure to bring these along to your first appointment. If you have dental insurance, bring your card and any necessary forms to help expedite the claims process. Also make sure to bring along a photo identification card.
If you are taking any medications, please make a list of those as well as dosages so we can include this in your medical history. Be sure to alert us to any medical conditions or allergies you have.
If you have had x-rays taken recently at another dental office, you can ask them to forward a copy to our office prior to your appointment. Or you can pick them up and bring them along to your appointment.
Think about the cosmetic goals you might have for your smile and feel free to jot some notes for ease of discussion with Dr. Hema. We also welcome your questions, so be sure to write them down and bring those along as well.
Your Dental Examination
As part of your initial examination, the following areas will be addressed:
- A thorough examination of your teeth and gums
- A periodontal evaluation
- An oral cancer screening of the soft tissues of the mouth
If you have made arrangements ahead of time, you might also undergo a teeth cleaning at your first appointment.
